Case Management is the foundation for all services delivered or coordinated by Horry County Disabilities and Special Needs. Horry County Disabilities and Special Needs has been providing exceptional case management services since 1984. We strive to assist families in identifying the supports and services they need utilizing the most person centered planning process. Once service needs are identified, the case manager will work directly with the individual and the family in addressing those needs thru referrals and linking to additional services.
